现场去Direction centrale de la police judiciaire d'Haïti (DCPJ) inPort-au-Prince办理

（2）居住在海的境外：

委托境内的朋友或亲属去Direction centrale de la police judiciaire d'Haïti (DCPJ) inPort-au-Prince代为办理

二、申请材料：

-A letter from Immigration, Refugees andCitizenship Canada stating that you must get a Haitian police certificate

-Four copies of a recent identificationphotograph

-A copy of the first page of your passport(page with name and passport number)

-A copy of your birth certificate (“Acte denaissance”, if you were born in Haïti)

-Your fingerprints 

-Fingerprints must be taken at apolice station in your place of residence (subject to local rules and procedures)or at your nearest Haitian consulate.

Special notes and considerations

Police certificates issued by regionalpolice offices such as « certificats de bonne vie et mœurs » and« certificats de tribunaux de première instance » are notacceptable.
